If you do not see your insurance company here, please call and we will do all that we can to get information on becoming providers with them. At your initial visit with us, we will contact your insurance company to obtain your specific coverage and requirements for physical therapy. We will make every effort to comply with your insurance company’s requirements to ensure proper coverage.
